These terms cover two things: using this website, and using the clinic. Please read them before you book. By using this website or booking an appointment with us, you accept the terms that apply to you.
01Who we are#
This website is operated by Medicate Pharma Ltd (company number 16277720), which trades as Burntwood Clinic at Chasetown Pharmacy, 23 High Street, Chasetown, Burntwood, Staffordshire WS7 3XE. Burntwood Clinic is the private clinic at Chasetown Pharmacy, a GPhC registered pharmacy (premises number 1038499).
Our Superintendent Pharmacist is Nimesh Vansia (GPhC 2086824). You can check our registrations yourself — see the regulatory information page.

Everything on this website is general information about our services. It is not medical advice, and it is not a substitute for a consultation. Do not delay seeking medical help because of something you have read here.
If you feel seriously unwell, call NHS 111. In an emergency — for example chest pain, severe breathing difficulty, heavy bleeding or sudden weakness — call 999.
Advice about travel health, vaccination and treatment depends on your own circumstances, and can only be given after an assessment by a pharmacist or prescriber.
03Website availability and accuracy#
We try to keep this website accurate and up to date, but information about services, availability and guidance can change — travel health requirements in particular change frequently. We do not promise that the website will always be available or free of errors, and we may change or withdraw any part of it without notice.
04Booking an appointment#
You can book online, by phone on 01543 682921, or in the pharmacy. A booking is confirmed when we confirm it to you.
Please tell us at the time of booking about anything that may affect your appointment — for example if you are pregnant or breastfeeding, have a condition affecting your immune system, take regular medicines, or need a longer appointment.

05Changing, cancelling and missed appointments#
If you need to change or cancel, please let us know as early as you can so that we can offer the appointment to someone else. Call 01543 682921 or email chasetownpharmacy@medicatepharma.co.uk.

We will also contact you as early as we can if we have to change or cancel your appointment, and we will offer you an alternative or a refund of anything you have paid for that appointment.
06Prices and payment#
Prices shown on this website are for guidance. The price that applies to you is the one we confirm with you before any treatment or supply goes ahead. If more than one appointment or dose is needed, we will tell you what the full course costs before you start.
- Payment is taken at the time of the appointment unless we agree otherwise.

- Most medical services provided by a registered health professional are exempt from VAT. Where VAT applies to an item, it is included in the price we quote.
We may change our prices from time to time. A change will not affect an appointment you have already booked and had confirmed at a stated price.
07A consultation does not guarantee that treatment will be supplied#
This is the most important term on this page. When you book, you are booking a consultation and assessment with a pharmacist. You are not buying a guaranteed supply of a medicine, a vaccine or any other treatment.
Our pharmacists and prescribers must act in your best clinical interests and within their professional standards. After assessing you, the pharmacist may decide that:
- a treatment is not clinically appropriate for you;
- a different treatment, or no treatment, is the better option;
- you should see your GP or another service first;
- a test or further information is needed before anything can be supplied;
- a vaccine course cannot be completed in time for your travel date.
That decision rests with the clinician and is final. We will always explain why, and what we suggest you do next.

08Your responsibilities#
So that we can look after you safely, you agree to:
- give us accurate and complete information about your health, your medicines, your allergies and your travel plans;
- tell us about any change in your health between appointments;
- bring any relevant records, letters or test results you have, including your vaccination records;
- follow the aftercare advice you are given, and contact us or NHS 111 if you are concerned afterwards;
- treat our team with respect. We may refuse or end an appointment where someone is abusive or threatening.
09Medicines, vaccines and prescriptions#
Many of the treatments provided at the clinic are prescription-only medicines. They can only be supplied after an assessment by a prescriber, and only where clinically appropriate. We cannot supply a prescription-only medicine on request.
We do not advertise prescription-only medicines to the public. Where this website describes a service, it is describing the consultation and assessment, not offering a named medicine for sale.
If you experience a side effect from any medicine or vaccine, you can report it to the MHRA through the Yellow Card scheme, and please tell us as well.
10Your records and your privacy#
We keep a clinical record of your appointment. How we handle your information, and your rights over it, are set out in our privacy policy.
11Intellectual property#
The content of this website — text, images, layout, graphics and branding — belongs to Medicate Pharma Ltd or is used with permission. You may view it, and print or download extracts for your own personal use.
You may not copy, republish, sell or use any part of it commercially, or remove any notice of ownership, without our written permission. Any trade marks shown belong to their respective owners.
12Links to other websites#
Where we link to another website — for example the NHS, TravelHealthPro, the GPhC register or a mapping service — we do so because we think it is useful. We do not control those sites and are not responsible for their content or their privacy practices.
13Our liability#
Nothing in these terms limits or excludes our liability for death or personal injury caused by our negligence, for fraud or fraudulent misrepresentation, or for anything else that cannot lawfully be limited or excluded.
Subject to that:
- this website is provided on an 'as is' basis, and we do not accept liability for loss arising from relying on general information on it;
- we are not liable for indirect or consequential loss, or for loss of profit, revenue or opportunity, however it arises;
- we are not liable for a failure caused by something outside our reasonable control.
If we provide a service to you as a consumer, your statutory rights — including the right to services carried out with reasonable care and skill — are not affected by these terms.

15Changes to these terms, and governing law#
We may update these terms. The version that applies to your appointment is the version published when you booked. The date of the current version is shown at the top of this page.
These terms, and any dispute arising from them or from the services we provide, are governed by the law of England and Wales, and the courts of England and Wales have exclusive jurisdiction. If you live in Scotland or Northern Ireland you may also bring proceedings in your own country's courts.
